In order
for us to serve you on your insurance and financial services needs, you have entrusted us with non-public, personal information.
We are committed to safeguarding your privacy; and, in compliance with Title V of the Gramm-Leach-Bliley Act (GLBA) and the

and claims experience reporting companies; and from credit reports or other investigative consumer reports.
INFORMATION WE
MAY DISCLOSE:
In the course of our general business practices, we may disclose the information that we collect about you or others
without your permission, in accordance with the law, for the following reasons: to enable a third party to perform a business,
professional or insurance function for us; to an insurance or financial institution, agent, broker or credit reporting agency
to perform a function in connection with a transaction that you have requested or to detect or prevent criminal activity,
mis-representation or fraud in connection with a transaction; to a regulatory authority, law enforcement or other governmental
authority in order to protect our interests in preventing or prosecuting fraud, or if we believe that you have conducted or
plan to conduct illegal activities; to a group policyholder for the purpose of reporting claims experience or to assist in
handling a claim on your behalf; to another entity or person as required by law or under subpoena to produce information;
or to a third party to assist us in marketing our own product or services. We prohibit any third party from further
sharing such information.
We do not sell or otherwise provide lists of our customers to third parties to market any non-financial
products or services.
YOUR RIGHT TO ACCESS AND AMEND YOUR PERSONAL INFORMATION:

that we record about you. You also have the right to request corrections to the information in our possession.
Any request for information must be submitted in writing to the office listed in this privacy policy and must include your
name, social security number and that recorded information that you would like to access. We may charge a reasonable

requested. If you are requesting a correction to your information, we shall make that correction within 30 days or notify
you that we refuse to do so and the reasons for the refusal, which you will have an opportunity to challenge.
OUR PRACTICES
REGARDING INFORMATION CONFIDENTIALITY AND SECURITY:
We restrict access to non-public, personal information about you to those employees
who need to know that information in order to provide products or services to you. We maintain physical, electronic
and procedural safeguards that comply with federal regulations to guard your information.
OUR POLICY REGARDING DISPUTE RESOLUTION:
Any controversy
or claim arising out of or relating to our privacy policy, or the breach thereof, shall be settled by arbitration in accordance
with the rules of the American Arbitration Association, and judgment upon the award rendered by the arbitrator(s) may be entered
in any court having jurisdiction thereof.
RESERVATION OF RIGHTS TO DISCLOSE INFORMATION IN UNFORESEEN CIRCUMSTANCES:
In connection
with the potential sale or transfer of its interests, our agency reserves the right to sell or transfer all of your information
to a third party entity that concentrates its business in a similar practice or service; agrees to be our successor in interest
with regard to the maintenance and protection of the information collected; and agrees to the obligations of this privacy
statement.
INSURANCE COMPANIES AND OTHER FINANCIAL PRODUCT PROVIDERS:
As an independent insurance agency, we place the product
or service that you have requested with an insurance company or other financial institution. In accordance with the
law, each of those companies should provide you with their privacy policy. We urge you to review those policies carefully
to determine if you need to take any additional steps to prohibit any release of your non-public, personal information.
